Order Forms
When you are ready to order, print and complete the appropriate order form(s). Then fax or email the form to us. If you are a city or county using a Purchase Order system include a copy of the purchase order and we’ll send an invoice for payment. Once your order is received, you will be contacted for next steps.

Payment Options
Pay by Check
- Mail the completed Shelter Pro Form and payments to the address on the order form.
Pay by Credit Card
- Payments via credit and debit cards require a 3.6% handling charge.
- If this is a new order, fax or email the completed order form to us. Contact information is on the Shelter Pro Order Form.
- Obtain your total order payment from the completed Shelter Pro Order Form or from our invoice. Then multiply this amount by 1.036 to determine the total amount including the 3.6% handling charge. This will need to be entered in the ‘Price per item’ during the payment process.
- Once payment is verified and the order form is received, we will contact you to complete the purchase. Click the “Pay Now” button below to complete the online payment process.
Bill me
- Fax or email the completed Shelter Pro Order Form. If your order will be paid by a purchase order, be sure to send a copy of the PO. Our mailing address, email, and fax will be on the order form.
